Applicants for a license under this subchapter shall file with the Town Clerk a written, sworn application in triplicate, signed by the applicant, giving the following information:
(A) Name and address of the applicant (both domiciliary and local), address, age, date and place of birth.
(B) The names and addresses of all partners, if a partnership, and the names and addresses of all officers and stockholders, if a corporation.
(C) A complete list of all prior arrests, if any, and all convictions, whether felonies or misdemeanors, of the applicant, and of all persons having any interest in the business of the applicant.
(D) The location where the applicant proposes to conduct his or her tattooing business.
(E) A copy of the applicant's tattoo parlor/shop registration with the Rhode Island Department of Health plus a copy of the Department of Health registration of each tattoo artist that will be tattooing in the applicant's establishment. All such registrations must be up-to-date.
