§ 113.05 APPLICATION.

   In addition to the information required by R. I. Gen. Laws § 5-11.1-5. the application must include:

   (A)   If the applicant is a business entity, all trade names and fictitious names used by the MFE;

   (B)   The current email address and phone number of the operator of the MFE;

   (C)   A completed taxpayer status affidavit;

   (D)   Address where MFE is located when not operating; and

   (E)   Any identifiable information regarding presence on the internet, including, but not limited to, website address and social media identifiers for Facebook, Twitter and/or Instagram.
