(A) Submission of applications. Proposals to collocate small wireless facilities are to be submitted to the Town Clerk by paper or electronic format as selected and required by the town. The small wireless facility permit application must be made by the wireless service provider, or its duly-authorized representative, and contain the following at a minimum:
(1) The applicant's name, address, telephone number and e-mail address;
(2) The names, addresses, telephone numbers, and e-mail addresses of all consultants, if any, acting on behalf of the application with respect to the filing of the application;
(3) A general description of the proposed work and the purposes and intent of the small wireless facility;
(4) Verification from an appropriate professional that the small wireless facility will comply with all applicable codes;
(5) Site plans, design plan(s) with dimensions and sizes, manufacturer's specification sheet(s), material type.
(B) Consolidated applications. An applicant may, in its discretion, file a consolidated application and receive a single permit to collocate small wireless facilities at multiple locations.
(C) In the event the application is incomplete, the town shall notify the applicant within 14 days after the initial submission that the application is incomplete and state what information that is lacking.
(D) Routine maintenance and replacement. An application and permit, other than a public right-of-way work permit if applicable, is not required for routine maintenance or the replacement of a small wireless facilities with another small wireless facilities that is substantially similar or smaller in size, weight, and height. Written authorization to proceed must be obtained from the town prior to proceeding with routine maintenance or replacement on a location where the town or another provider has placed equipment or facilities, such authorization may not be unreasonably withheld.
