901.01 IMPACT EVALUATION REPORT.
   In order for the Village to determine what services, if any, it will provide to territory proposed for annexation upon annexation, as required by Ohio R.C. 709.03(B), an impact evaluation report shall be provided as follows:
   (a)   The impact evaluation report shall include the following:
      (1)   Name, address and phone number of applicant.
      (2)   Legal description of property as recorded in Geauga County Recorder’s office.
      (3)   Existing and proposed uses of the property.
      (4)   Impact evaluations of the territory proposed for annexation based upon the proposed use of the territory as follows:
         A.   For residential development only, a population impact evaluation which shall be summarized in text, with an analysis relating to the development proposal(s) and shall include:
            1.   Projected total population.
            2.   Projected population of each development section.
            3.   Age group estimates (0-4, 5-13, 14-18, 19-35, 36-50, 51- 65 and 65+)
            4.   Projected populations by dwelling unit type.
            5.   Family projections by age (head of household, wife of head, children under 18).
         B.   A market report which shall contain an analysis indicating the project marketability of the development in regard to effective demand specifically relating to the site to the Village. Any public and/or subsidized housing shall be identified to include an explanation of the assistance program and the number of units affected.
         C.   A traffic impact evaluation which shall be summarized in text with an analysis relating to the development proposal:
            1.   The projected number of motor vehicular trips to enter or depart the site shall be estimated for an average day and peak hours; and
            2.   The projected traffic flow pattern shall be described and related to a road and parking map including vehicular movements at all major existing and proposed intersections; and
            3.   The impact of this traffic upon existing streets shall be evaluated in relation to road capacities using both current and redesign criteria; and
            4.   The report shall contain an analysis of the proposed improvements to all roads specifying right of way and driveway widths, and an explanation of typical road construction showing base and subbase. The clear sight triangle dimensions of all major intersections shall be specified.
         D.   A utilities impact evaluation which shows the probable impact from utility needs shall be determined and shall include:
            1.   Existing capacity of sewage treatment facilities; and
            2.   The estimated impact of water demands upon existing service facilities shall be evaluated with the estimated daily and peak hour volume demand; and
            3.   The report shall contain an analysis of provisions for gas, electricity, telephone mail service and refuse storage and collection. Construction processes shall be specified to include common trenches where feasible.
         E.   A drainage impact evaluation which shows the probable impact resulting from the flow of storm water shall be determined, including:
            1.   The projected maximum volumes at the collection point from each drainage system shall be calculated on the basis of a twenty-five year storm; and
            2.   The estimated impact of storm water upon existing service facilities shall be evaluated in relation to existing storm water capacities, and shall conform to existing Soil Sediment Pollution Control Regulations; and
            3.   An evaluation of the flushing action of flows on any pipe to be utilized in the drainage system to assure the flushing of discharged materials; and
            4.   An evaluation of the quality of storm water to be discharged into any pipe through the storm drainage system detailing how this discharge shall not lower the quality of the water in the system; and
            5.    The report shall contain an analysis of all improvements including off-site conditions to facilitate the flow of storm water. Construction process shall be specified for systems.
   (b)    Two copies of the impact evaluation report shall be provided to the Planning and Zoning Commission which shall refer one copy to the Village Engineer for a report and recommendation as to the impact of the annexation upon existing Village utilities. Upon receipt of the Engineer's report, and after review of the complete impact evaluation report, Planning and Zoning shall make its recommendation to Council.
   (c)    Costs for the services of the Village Engineer to review the impact evaluation report shall be the responsibility of the petitioner for annexation. A one hundred dollar ($100.00) deposit shall be required at the time of the submission of the impact evaluation report. If the actual cost for the services of the Engineer's review of the report exceeds one hundred dollars ($100.00), the Engineer shall provide an estimate of such additional costs and the applicant shall deposit that additional amount prior to further review by the Village. After all review costs have been deducted, and after the Planning and Zoning Commission has made its recommendation to Council, the balance of the deposit shall be returned to the petitioner.
