1224.05 FINAL PLATS; FORM AND INFORMATION REQUIRED.
   (a)   Form. The final plat shall be clearly and legibly drawn in India ink on tracing cloth. The size of the map shall be not less than twelve inches by eighteen inches. The map of a subdivision containing six acres or less shall be drawn at a scale of one inch equals fifty feet. All other subdivisions shall be drawn at a scale of one inch equals 100 feet, unless otherwise required by the Planning Commission.
   (b)   Information and Data. The following information and data shall be shown on the final plat:
      (1)   The name of the subdivision, its location by section, town, range, township, county and state, the scale, the date and the north point.
      (2)   All plat boundaries with lengths of courses to hundredths of a foot and bearings to half minutes. Error of closure shall not exceed 1 to 10,000.
      (3)   The exact location and width of all streets or other public ways within or adjacent to the plat.
      (4)   Bearings and distances to the nearest established street, municipal, township, county or section lines, or corners or other recognized permanent monuments, which shall be accurately described or located on the plat.
      (5)   The exact layout, including street and alley lines with names, bearings, angles of intersection and widths; lengths of all arcs, radii, internal angles, points of curvature and tangent bearings; and all easements for rights of way provided for public utilities and any limitations on such easements.
      (6)   All lot numbers and lines, with accurate dimensions to hundredths of a foot, with bearings or angles to the street line.
      (7)   An accurate outline of all property which is offered for dedication for public use or reserved for the common use of the property owners, with the purpose indicated thereon.
      (8)   Building setback lines, with dimensions.
      (9)   The accurate location and the material of all permanent reference monuments.
      (10)   Protective covenants, if any, to be shown on the plat.
      (11)   Certification by a registered engineer or surveyor to the effect that the plan represents a survey made by him or her and that all monuments shown thereon actually exist and that their location, size and material are correctly shown.
      (12)   An acknowledgement by the owners of their adoption of the plat and the dedication of the streets and other public ways.
      (13)   The names and locations of adjoining subdivisions and the locations and ownership of adjoining unsubdivided property.
