Applications for the license required by Section 808.02 shall be made to the Chief of Police upon such blank forms as shall be prescribed therefor. On such application, the applicant shall state his or her name; whether an individual, partnership or corporation, and, if a partnership, the names and addresses of the partners composing the same; the addresses at which it is desired to maintain and operate the billiard table or pool table; the nature of the business conducted at such place; the make, name, model serial number and other identifying information with reference to the particular billiard table or pool table which it is desired to keep for gain or hire; the name and address of the owner of such billiard table or pool table; and such other and further information as the Chief of Police may require. No person who is not eighteen years of age or older shall make an application for such a license.
