SECTION 15.1003.  APPLICATION FOR AMENDMENT.
   A.   The petitioner shall file an application for an amendment on forms provided by the Zoning Administrator, who shall forward a copy without delay to the Village Clerk for processing the notice therefor.
   B.   The application for an amendment shall contain the following information, as well as such additional information as may be prescribed by further rule of the Zoning, Planning and Appeals Commission.
      1.   In the event any such amendment is for a change in the text of this Chapter, or any part thereof, such application shall contain the following:
         a.   The particular Article, section, or part thereof, to be amended; and
         b.   A brief statement of the amendatory language sought.
      2.   In the event such amendment will constitute a rezoning of certain land and territory, such application shall contain the following:
         a.   The present zoning of the subject property;
         b.   A brief statement of the amendment that is sought;
         c.   The legal description of the subject property; and
         d.   The commonly known location (address) of the subject property.
   C.   At the time of filing, applicants having a proprietary interest in the property for which an amendment is being sought shall provide the Zoning Administrator with:
      1.   Proof of ownership, proof of authority on behalf of the owner, or current contract to purchase or lease the subject property, since only persons owning or having interest in the subject property may file an application to amend the zoning classification of such land; and
      2.   Payment of filing fees of Two Hundred Fifty dollars ($250).
