761.02 PERMIT REQUIREMENTS.
   (a)   Prior to operating a tattoo or body piercing establishment within the City of Mentor, every operator shall obtain a permit to operate from the City. The permit to operate shall expire December 31 following the date of issuance and shall be renewed annually in December before the first day of January of the next year.
   (b)   An annual permit fee of Three Hundred Dollars ($300.00) shall be paid by each tattoo or body piercing establishment operating within the City of Mentor. An application for permit to operate a tattooing or body piercing establishment shall be accompanied by the fee in the amount of Three Hundred Dollars ($300.00). Any change of ownership shall require an application and permit with payment of associated fees.
   (c)   It shall be unlawful for any person, firm or corporation to engage in the business of operating a tattoo or body piercing establishment within the City of Mentor without a permit issued by the City of Mentor to engage in such business. Such a permit shall be displayed in a conspicuous place within the establishment.
   (d)   The operator of the tattoo or body piercing establishment must complete an annual training session at the Board of Health which includes all health related issues incident to such establishments.
   (e)   The Health Commissioner may inspect, make inquiry about and test any equipment, both new and used, to be utilized in the tattoo or body piercing establishment and same must be approved by the Health Commissioner.
   (f)   The Heath Commissioner shall assist the City of Mentor in conducting annual and other periodic inspections, as may be necessary, or any tattooing or body piercing establishment for the purpose of determining whether or not said establishment and the persons performing the tattooing or body piercing therein are in compliance with all applicable health provisions contained within this regulation and other pertinent regulations of the Health District, City of Mentor and the State of Ohio. It shall be unlawful for any person or operator of a tattooing or body piercing establishment to willfully prevent or restrain the Health Commissioner from entering any tattooing or body piercing establishment for the purpose of inspecting said premises, after proper identification is presented to the operator.
