(a) Application for a license to display a mechanical or electronic amusement device must be made to the Director.
(b) Such application shall be made by the owner of the mechanical or electronic amusement device. The application shall state the following:
(1) The name and address, age and birth date of the owner of the machine or machines, or if a firm, partnership, corporation or association, then the names and addresses of the officers and any additional information requested by the Director.
(2) Any other information deemed relevant by the Director.
(c) The license required herein shall not be transferred to any other person.
