Any person, including an applicant for a massage establishment permit, who engages in the practice of massage, shall file an application for a masseur or masseuse registration with the City of Mentor upon a form provided by the City and shall pay a nonrefundable filing fee of fifty dollars ($50.00), receipt to be-attached to the application filed with the Director of Planning.
