(A) Applications for registration shall be made in such form and in accordance with such instructions as may be provided by the City Housing Official and shall contain at least the following information:
(1) The name, address and telephone number of the applicant;
(2) The names, addresses, telephone numbers and dates of birth of all owners of the residential rental structures;
(3) The name, local address, telephone number and date of birth of the agent;
(4) The number of residential rental structures at each site, the address of, and number of, residential rental units in each residential rental structure, and the number of occupants in each rental unit; and
(5) An authorization appointing an agent by the owner - signed by both.
(B) A registration fee for each site where residential rental structures are located shall be paid at the time of registration. No post office boxes will be accepted as legal addresses. Upon registration, the Building Official or authorized representative shall inform applicants of certificate of compliance requirements. The fee for each registration shall be established by resolution by the City Council. The owner shall be responsible for notifying the city of any change of address of either owner or the responsible local agent.
