3-1-3: APPLICATION FOR LICENSE:
All applications for licenses shall specify:
   A.   The name of the person desiring a license.
   B.   The kind of license desired, stating the business, calling or profession to be performed, practiced or carried on.
   C.   The place where such business, calling, trade or profession is to be carried on. (1976 Code § 11-1-4)
