§ 115.19 BID FORMS.
   (A)   The County Clerk shall likewise prepare and furnish to prospective bidders upon request a form for bidding on the non-exclusive franchise. No bid will be considered unless it is filed on such form. The county reserves the right, at its option, to waive informalities or irregularities in bids.
   (B)   The said form shall require the bidder to furnish the following information:
      (1)   A statement of the ownership of the bidder as follows:
         (a)   If the bidder is a corporation, the state and date of incorporation, the name and address of each director and officer, and the name and address of each shareholder owning in excess of 20% of the stock ownership of such company;
         (b)   If the bidder is a partnership, association, or joint venture, the name and address of each general partner or joint venturer, the interest owned by each general partner or joint venturer and the type of such partnership, joint venture or association (i.e., whether general, special or otherwise); and
         (c)   If the bidder is an individual or sole proprietorship doing business under an assumed name, the name and address of the bidder or of all persons or firms making up the proprietorship and the interest owned by each person or firm.
      (2)   Whether such bidder is the owner of any interest in other CATV franchises or systems, and the number, location and address thereof;
      (3)   Whether such bidder is the owner of any interest in any license issued by the Federal Communication Commission, and if so, a description of the license, and whether such bidder has ever been convicted of any violation of any law or regulation with respect to such license;
      (4)   A statement that the bidder has read and is familiar with the contents of CATV Operation Rules and Technical Standards, published by the Federal Communications Commission;
      (5)   A statement of the bidder’s technical qualifications for the construction and operation of a CATV system and a statement showing the adequacy and feasibility of its construction arrangements; and
      (6)   A schedule of the rates to be initially charged for services rendered.
