§ 2.28.200 APPLICATION FOR AN APPEAL.
   A.   Procedures.
      1.   The Applicant must file a copy of the Appeal with the Code Enforcement Officer and the Board of Building Code Appeals’ Secretary/Coordinator. The Code Enforcement Officer will provide the Board with a copy of the entire file in the matter.
      2.   An appeal shall provide the following information:
         a.   The name and address of the applicant.
         b.   The location of the property.
         c.   Copy of the Code Enforcement Officer’s letter outlining reasons for the decision being appealed.
         d.   Identification of the ordinance provisions in dispute, and the reason given by the enforcing officer for the decision being appealed.
         e.   A description of the proposed use, including a plot plan if applicable.
         f.   A specification of the grounds for the appeal.
      3.   The appeal shall be signed by the applicant or his attorney with their address and telephone number.
