Any person who files a complaint for foreclosure involving real property located within the Village of McDonald shall file an Affidavit of Facts with the Village within ten (10) days of filing the foreclosure complaint. The Affidavit of Facts shall be in written form as prescribed by the Village of McDonald, and shall contain the following information:
(a) The real property address and parcel number;
(b) The current owner of the real property and the current mortgage holder of the real property, as reflected in the records of the Trumbull County Recorder, including reference to the volume and page number in the Trumbull County Recorder’s office where said information is contained;
(c) The name, address, telephone number, and fax number of both the person filing the foreclosure action and the plaintiff in the foreclosure action;
(d) The date the foreclosure action was filed and case number assigned by the Trumbull County Common Pleas Court;
(e) Whether the real property is vacant or, when the real property becomes vacant subsequent to the filing of the foreclosure action; and
(f) The name, address and telephone number of the person responsible for maintaining the real property during the pendency of, and subsequent to, the foreclosure action.
