(a) Application for a license to operate a massage establishment, including a renewal license, shall be made pursuant to this chapter on a form provided. Each application shall include a filing fee of five hundred dollars ($500.00) which shall not be refundable.
(b) The application for a license to operate a massage establishment shall set forth the proposed place of business and facilities therefor.
(c) The application for a license to operate a massage establishment shall set forth the following as to each and every employee and massage technician working for, or to be imminently working for, the establishment:
(1) Name, residence address and social security number; and
(2) A physical description setting forth the applicant's weight, height, hair color and color of eyes. (Ord. 2398-85. Passed 5-21-85.)
