Renewal of a license shall be on forms prescribed and furnished by the city clerk. Renewal forms shall be submitted thirty (30) days prior to the expiration of the current license. The required license filing fees and an affidavit verifying that the information contained in the original application is unchanged must accompany a renewal form. (Ord. 938, 12-17-2015)
