§ 111.03 PERMIT APPLICATION.
   (A)   Any person desiring to secure such a solicitor’s permit shall apply therefor in writing over his or her signature to the Village Hall and Police Department located at 520 Depot Street on forms provided by the village, and such application shall state:
      (1)   The name and address of the applicant;
      (2)   Name, address and phone number of the person, firm or corporation by whom employed;
      (3)   The number of years the person, firm or corporation by whom the applicant is employed has been in business;
      (4)   The nature or character of the goods, wares or merchandise to be offered by the applicant;
      (5)   The name, address, date of birth and driver’s license number of the applicant;
      (6)   The name, addresses, dates of birth and driver’s license numbers of any additional solicitors; and
      (7)   The models and license plate numbers of the vehicles to be used by the applicants or additional solicitors.
   (B)   Such application shall be accompanied by such credentials and other evidence of the identity, honesty and good character of the applicant as may reasonably be required by the President and Board of Trustees of the village as enforced by the Police Chief or representative of the Village Police Department. Such applicant shall pay to the village the fee as stated herein to cover the expense of granting and policing such permit.
