(A) Applications for all licenses shall be made to the village in writing on the form provided for that purpose. Each application shall contain, at a minimum, the following:
(1) Business name;
(2) A description of the type of business;
(3) Address of proposed business;
(4) Business phone number; and
(5) Identification of the business owner, manager, operator or responsible person, including residential address and telephone number.
(B) Any person desiring a business license shall file an application with the village not less than 30 days prior to the time said person desires to commence the business operation.
