§ 113.07 DAILY REPORTS.
   (A)   It shall be the duty of every person conducting or operating any home regulated by this chapter to make a report to the Building Director daily, by telephone and confirmed in writing immediately thereafter, of all epidemic, communicable or contagious diseases. Such daily reports shall contain the names and residences of all persons suffering from any such diseases, together with such other information as may be required by the Building Director. A copy of all such reports shall also be mailed to the county and state health departments.
   (B)   It shall be the duty of every person conducting or operating any home regulated by this chapter to report to the Village Police Department by telephone any unusual occurrences, such as unaccounted for disappearance of controlled substances and syringes, accidents resulting in severe personal injury or death, attempted suicides, suicides, and sudden unexplained deaths.
