Any person desiring a license for any such motor vehicle shall prepare an application upon a form provided by the Director of Finance, setting forth the name and address of the applicant, a description of the motor vehicle for which the license is desired, its state license number, if any, and the place where such motor vehicle is to be kept by such applicant at such place, and such other information as may be prescribed.
