   (a)   Permit for fireworks under Section 1519.02, subsection (c), twenty-five dollars ($25.00).

   (b)   An indemnity bond per Section 1519.02(c) in the amount of at least five million dollars ($5,000,000) or proof of insurance coverage of at least five million dollars ($5,000,000) for liability arising from injury, death, or loss to persons or property emanating from the fireworks exhibition.
