767.709 TELECOMMUNICATIONS, RIGHT OF WAY PERMITS.
   (a)   Per Exhibit A, Section 2.3, the Permittee shall pay to the Village an Acceptance Fee based on a formula of one dollar ($1.00) per linear foot of street traversed and one thousand dollars ($1,000) per underground public street crossing to cover costs incurred by the Village in reviewing, enforcing and granting the Permit.
   (b)   Per Exhibit A, Section 6.3, the Permittee shall maintain a certificate of insurance for public liability and property damaged that protects the Permittee and the Village, its officers, agents, employees and appointed and elected officials. The insurance shall provide coverage at all times for not less than five hundred thousand dollars ($500,000) for bodily injury to any one person and one million dollars ($1,000,000) bodily injury aggregate per single accident or occurrence and five hundred thousand dollars ($500,000) for damage to any single property, and one million dollars ($1,000,000) for property damage aggregate per since accident or occurrence, plus costs of defense, or in such other amounts as are acceptable to the Village and as provide substantially equivalent coverage.
   (c)   Per Exhibit A, Section 6.4(1), Permittee shall provide the Village a construction performance and construction completion bond in an amount equal to ten percent (10%) of the applicable construction cost. Such amount shall not exceed two hundred thousand dollars ($200,000).
   (d)   Per Exhibit A, Section 6.4(2), Permittee shall deposit with the Village a Performance Bond in the form of a surety bond or letter of credit in the amount of fifty thousand dollars ($50,000).
