1365.02 ISSUANCE OF CERTIFICATE OF INSPECTION.
   (a)    Application for a Certificate of Inspection as required by this chapter shall be made upon forms provided by the Building Commissioner from the Cuyahoga County Board of Health.
   (b)    The Certificate of Inspection shall contain at least the following information:
      (1)    The street address or other identifying characteristics of the dwelling structure or unit.
      (2)    The name and address of the owner of the dwelling structure or unit.
      (3)    Listing of any noncompliance items contained within the private sewage systems at the time of such inspection.
   (c)    The Certificate of Inspection shall be valid for a period of 120 days from the date of the inspection required herein.
