Every application shall be made to the City Clerk, on a form provided by the Clerk. It shall be accompanied by payment for the prescribed fee. The fee shall be refunded if the license is not approved. The applicant shall provide any information as may be required on the application or form. No person shall make a false statement in or concerning any application.
