§ 111.19 MANAGER’S LICENSE.
   (A)   (1)   All licensees pursuant to this chapter shall not open for business without having obtained a manager’s license for an individual who shall be responsible to manage and direct the business activities of the licensee and shall be the legal representative of the licensee to the village regarding any and all matters affecting the village’s issuance of said license. The manager may or may not be the owner but must be an individual and meet the qualifications and restrictions of the licensee as provided in § 111.15, except does not need to be a resident of the village and shall submit an application in writing.
      (2)   The application must be verified by the owner of the license and at a minimum, include the following:
         (a)   The name, date of birth, residence address, current telephone number and Social Security number;
         (b)   A copy of any management agreement;
         (c)   A statement as to whether the applicant has ever been convicted of a felony, gambling offense, violation of a law concerning the manufacture, possession or sale of cannabis, narcotics or controlled substances, or violation of law concerning the manufacture, possession or sale of alcoholic liquor;
         (d)   A statement as to whether or not the applicant has held a license for the sale of alcoholic liquor in any state which has been revoked and the reasons for revocation;
         (e)   A statement that the applicant will not violate any laws of the state, United States or the village in the conduct of business; and
         (f)   Such other information as the Liquor Commissioner may require including, but not limited to, fingerprinting.
   (B)   Persons applying for a manger’s license are subject to a criminal background investigation to determine eligibility for such license. The authorization for such is deemed granted by the execution of the application. The investigatory agency shall furnish all records of convictions, unless expunged, to the Liquor Commissioner.
