(A) Anyone who discharges industrial wastes into the city’s sewerage system shall, upon the written request of the Board of Public Works and Safety, fill out and file with the city within 90 days an industrial wastes questionnaire to be furnished by the city, in which he or she shall set out the quantity and characteristics of the wastes discharged into the city’s sewerage system. Anyone wishing to establish a new connection to a public sewer for the purpose of discharging industrial wastes must also fill out and file a questionnaire, which shall include actual or predicted data relating to the quantity and characteristics of the wastes to be discharged.
(B) When special circumstances would make complying with the 90-day time schedule an unreasonable burden on the person, an extension of time, not to exceed 90 days, may be granted by the Board of Public Works and Safety upon presentation of a proper application.
