§ 111.04 ITINERANT VENDOR REGISTRATION.
   (A)   Registration under this chapter will be on a form provided by the City Clerk containing at least the following information:
      (1)   The full name of the registrant;
      (2)   The registrant’s permanent home and business addresses;
      (3)   The name and address of the company, if any, which the registrant represents;
      (4)   The registrant’s date of birth and driver’s license number;
      (5)   A detailed description of the goods or services to be offered by the registrant;
      (6)   A description of any vehicle to be used by the registrant including the make, color, and license plate state and number;
      (7)   If the sales involve taking orders for future delivery, the address from which orders will be shipped, and the method of delivery;
      (8)   The number and date of issuance of the state sales tax permit under which the registrant operates;
      (9)   Whether the registrant has ever been convicted of any felony, or any form of fraud, theft, or other crime involving moral turpitude. If so, the type of crime and date and place of conviction; and
      (10)   A statement under oath by the registrant that all information provided on the registration form is true and correct.
   (B)   Registration shall be accompanied by a fee of $20.
   (C)   Upon filing of a completed registration form and payment of the registration fee, the City Clerk will issue a registration certificate to the registrant. The certificate may consist of a copy of the registration form that includes the City Clerk’s notation of filing. The certificate will contain a statement that the certificate in no way constitutes an endorsement of the registrant’s activities.
   (D)   Registration under this section shall be valid for a period of three months.
