§ 57.021 PERMIT APPLICATION.
   Before any permit is issued under provisions of this subchapter, an application shall be filed with the Director of Public Works/City Engineer stating the place; the amount and purpose of the contemplated work; the time when the work is proposed to be done; the time within which it will be completed; and in what street, avenue, alley or public place such excavation is to be made or work done, and its location thereon. Each application must contain a stipulation that the applicant will, within the time limit therein, fill any excavation or trench in a proper and workmanlike manner and will restore the street, avenue, alley or public place to its proper condition, and that if the applicant fails or neglects to fill such excavation or trench properly and restore the street, avenue, alley or public place to its proper condition within such time, the Director of Public Works/City Engineer may do so or cause the work to be done and charge the applicant with the cost and expense thereof.
