(A) Application for license hereunder shall be filed in writing with the City Clerk and shall specify:
(1) The name and address of the applicant and, if a firm, corporation, partnership or association, the principal officers thereof and their addresses.
(2) Whether the applicant is a citizen of the United States.
(3) The place where the business of the applicant is to be carried on.
(4) The name of the manufacturer of the game and the type of game, the number of games to be used in the city, and the serial or factory number of each machine.
(B) The application shall be referred to the Council for investigation of the character of the applicant, the premises in which it is proposed to carry on the business of the applicant, and the type of game to be used. Such license shall be non-assignable and non-transferable, and in the case of a proprietor shall apply only to the premises for which such license is issued.
