§ 52.15 APPLICATION FOR WATER SERVICE.
   (A)   A written application for water services must be completed and delivered to City Hall. The application must include evidence of ownership of the property in question such as by a copy of a deed, title policy, or property insurance policy. The applicant must present with the application appropriate identification. When submitting their application, applicant(s) must present adequate identification. Identification must show the applicant’s name, date of birth, and residential or business address and no less than one form of photographic identification such as driver’s license or state identification card. A business entity must provide proof of the business entity’s existence and all principals of the business must be identified on the application. No less than one principal of the business must present the application and such principal will need to provide adequate identification and proof of his or her principal relationship with the business entity. No social security numbers or tax identification numbers will be requested in the application process. Water bills will not be sent to tenants. Owners will receive bills for all service accounts at only one mailing address.
   (B)   No owner seeking to establish a new city water and sewer account, shall be given water service until any and all delinquent water and sewer charges shall have been paid in full. A charge of $35 will be made for turning on water and renewing service.
