Application for the license shall include the following:
(A) Name and residence of applicant and any agents that applicant intends to employ;
(B) Make, model, year and license plate number for motorized vehicle applicant intends to use;
(C) Proof of valid driver's license for applicant and all agents intending to use the motorized vehicle;
(D) The particular goods to be sold;
(E) Proof of any necessary permits or licenses as required by the LaSalle County Health Department, Illinois Secretary of State and any other agencies under the state;
(F) No license shall be granted to any individual on the Illinois Sex Offender Registry;
(G) All applicants and agents of the applicant are subject to a criminal background check. Any applicant or agent of the applicant convicted of a felony shall require action by the City Council as to whether a license shall be granted; and
(H) Proof of valid automobile insurance.
