§ 117.29 APPLICATION FOR TAG DAY.

   Applications for such permits shall be made to the City Clerk and shall include the following information:

   (A)   Name of organization;

   (B)   Financial statement for organization;

   (C)   Purpose of tag day;

   (D)   Date upon which tagging or soliciting will be done;

   (E)   Area in which tagging or soliciting will be done;

   (F)   Names and addresses of persons who will be tagging or soliciting; and

   (G)   Such other information as may be pertinent and necessary.
