§ 98.036 APPLICATION FOR PERMIT.
   (A)   Applications for such permits shall be made to the City Clerk and shall state thereon the location of the intended improvement to be constructed or other work to be done, the name of the person doing the actual work and the name of the person for whom the work is being done.
   (B)   One copy of application to go to the Superintendent of Public Works for approval.
