(A) Any employee desiring to take FMLA leave should contact his or her supervisor as soon as possible.
(B) A request for FMLA leave for the birth of a child or placement of a foster or adopted child should be made at least before the leave can commence, or in emergency cases as soon as possible. Leave for any other reason allowed by the FMLA should be requested as soon as the employee knows the need for FMLA leave.
(C) Any employee taking leave for his or her own serious health condition or that of a statutory family member must submit a written medical certificate issued by the attending health care provider on the form which may be obtained from the employee’s supervisor or the Human Resource Department. Failure to provide such a certification within 15 days of making the leave request or commencing leave, whichever is sooner, will result in termination of the leave and a requirement that the employee immediately return to work, or face discipline under the city’s attendance policy.
