§ 35.114 MEDICAL INSURANCE BENEFITS WHILE ON FMLA LEAVE.
   (A)   The city will maintain in force the medical insurance benefits provided to the employee prior to taking FMLA leave, under the same terms and conditions as existed prior to leave. If the employee was required to pay a portion of the premiums for coverage, that obligation continues while on leave.
   (B)   Payment is expected to be made in the same amounts, and at the same time (i.e., each payroll date) as was made while working. If any payment is more than 30 days late, medical coverage will be canceled without further notice.
   (C)   An employee can elect not to continue medical coverage while on leave. If this election is made, the city will immediately offer COBRA coverage.
   (D)   If the coverage is continued while on FMLA leave, and the employee does not return to work at the end of the FMLA leave period, the city will bill the employee for the amount of premiums paid by the city during the leave period.
   (E)   No other employment benefits provided by the city are continued during FMLA leave. All such benefits are instead held in abeyance until the employee returns to work.
