All applications for a contractor's license shall be made in writing to the Building and Housing Department, upon forms furnished by the city. Each application shall be in accordance with Chapter 110 of this code and additionally state the name and business address of the applicant, whether it is a contractor, subcontractor or landscape and lawncare professional license application and if applicable, the trade, or trades involved who would be operating under such license and a statement that the applicant is familiar with all pertinent city ordinances, code and regulations including building codes and agrees to fully comply with same.
