§ 120.08 LICENSE SUSPENSION OR REVOCATION.
   (A)   After written notice by certified mail to the holder of the contractor license at his or her address listed under § 120.03 of this chapter; personal delivery or posting at the site in which the contractor is working and the opportunity for an informal hearing, any license issued under the provisions of this chapter may be suspended or revoked by the city for any violation of this code of ordinances or other actions including, but not limited to, making false statements on any application or failing to pay any taxes, fines or fees owed to the city. Any such suspension or revocation shall be in addition to any fine imposed for the conduct giving rise to the revocation or suspension. No person whose license has been revoked as herein provided shall be eligible for a new license for a one-year period from the date of revocation or suspension.
   (B)   Prior to suspension or revocation, the Building and Housing Department shall notify, in writing, the holder of the contractor license pursuant to division (A) above of the specific reasons for which the license is to be suspended or revoked and that the license shall be suspended or revoked at the end of ten days following service of such notice unless the license holder files a written request for a hearing in the office of the Building and Housing Department. Upon receipt of such request, the Building and Housing Department shall set a time and place for a hearing to be held within ten days after receipt of such request. After the hearing, the Building and Housing Department shall promptly issue a written decision to the holder of the contractor license concerning the matter. If no request for a hearing is filed within the ten-day period, the suspension or revocation of the license shall become final.
   (C)   No person whose license has been revoked two times previously or whose license and permit bond has been collected on in any amount two times previously shall be eligible for licensing for five years from the date of the last revocation.
