All applications for contractor’s license required in § 120.02 shall be made in writing to the Building and Housing Department, upon forms furnished by the city. Each application shall state the name and business address of the applicant, whether it is a contractor or subcontractor’s license application and if the latter, the trade, or trades involved who would be operating under such license and a statement that the applicant is familiar with all pertinent city ordinances, code and regulations including building codes and agrees to fully comply with same.
