Application for licenses for taxicabs or motor vehicles for hire shall be made by the owner upon blanks to be furnished by the City Clerk and the applications shall contain:
(A) The full name and address of the owner;
(B) The class of vehicle for which a license is desired;
(C) The length of time the vehicle has been in use;
(D) The number of persons it is capable of carrying; and
(E) The motor power thereof.
