The City Clerk shall keep a complete and public record of the issuance of each license and all renewals, suspensions and revocations thereof, which record shall be kept on file with the original application for the license. If a member of any cooperative association, the licensee shall give the name of the association and shall notify the City Clerk of any change in his or her membership or affiliation within 48 hours of the change.
