(A) No interment shall be made in any cemetery belonging to the city except as hereby provided.
(B) The person desiring to make an interment in any cemetery shall provide a burial transmit permit, in writing, signed by him or her, to the City Clerk, which shall contain, if obtainable, the full name of the deceased, his or her age and residence at the time of death, the sex and place of death, whether within the city or elsewhere, and the cause of death as certified by the medical attendant or coroner, and shall pay to the clerk the amount of the sexton's fees hereinafter prescribed.
