Except as provided in § 33.24, participation in the plan shall terminate in accordance with the contract(s) on the earliest of:
(A) Termination of the plan; or
(B) Non-payment of any required retiree contributions; or
(C) Death of the individual receiving benefits under the plan; or
(D) In the case of a spouse or dependent, the date the spouse or dependent ceases to be a spouse or dependent as defined in this chapter; or
(E) In the case of a spouse or dependent, the death of the retiree if the retiree elected a level straight life form of retirement at the time of retirement or if the retiree failed to nominate his or her spouse as beneficiary under an optional form of benefit, necessary to ensure coverage of the spouse or dependent; or
(F) In the case of a retiree, if that individual assumes employment elsewhere and that employer provides health coverage to its employees which does not substantially differ from that offered by the plan.
