(a) Each written application for a permit, as provided for by this chapter, shall be accompanied by a fee of one hundred twenty dollars ($120.00) and a bond or certified check on some solvent bank and in a sum sufficient to cover the costs of pavement replacement as required by the Director of Public Safety and Service.

(b) Applications for permits under this chapter shall be made in writing on forms to be provided by the Director of Public Safety and Service. Such application forms and permit forms may be altered by the Director from time to time as required.
