§ 152.03 WRITTEN APPLICATION.

   An owner shall apply for a rental permit by filing a written application with the office of the Building Department which contains the following information:

   (A)   Name of owner;

   (B)   Address of owner;

   (C)   Address of property to be inspected; and

   (D)   Name of tenant (if known at time of inspection).
