§ 95.03 LICENSES.
   (A)   License required.
      (1)   No person shall keep any dog over 6 months of age within the city without securing a lifetime license therefore from the City Administrator, who shall keep a record of all licenses issued and shall issue a tag for each license.
      (2)   A license shall be secured from the City Administrator within 10 days of obtaining a dog over 6 months of age, or within 10 days of the date when the dog becomes 6 months of age.
   (B)   Application for license. Applications for license shall be made on forms prescribed by the City Administrator, which forms shall set forth the following:
      (1)   The name, address, and telephone number of the owner; and
      (2)   The breed, age, and sex of the dog for which a license is sought.
   (C)   License fees; expiration. The lifetime license fee shall be set by Council resolution annually. Every license shall expire upon death of the dog or when the dog is no longer in applicant's possession. A lifetime license does not transfer to a different animal. The lifetime license expires on December 31 each year if current proof of rabies shots is not provided to the city.
   (D)   Rabies vaccination. Every application for a license shall be accompanied by a certificate from a qualified veterinarian showing that the dog to be licensed has a current certificate of rabies vaccination.
   (E)   Affixing tag.
      (1)   The owner shall be required to provide each dog with a collar or harness to which the license tag must be permanently affixed in a manner so that the tag may be easily seen, or have the license number tattooed inside the ear of the dog.
      (2)   The owner shall see that the dog constantly wears the tag.
      (3)   Tags shall not be transferable and no refunds shall be made on any dog license fee because of leaving the city or death of the dog.
