Persons seeking to obtain or renew a hotel operator's license shall file an application with the Village Clerk on forms to be provided by the village. The application must be completed and signed by the proprietor/owner. The application shall set forth, with the required attachments, the following information:
(A) Name, home address, business address, home phone number, business phone number, mobile phone number and facsimile number of the proprietor/owner;
(B) The street address of the hotel, permanent index number of land on which it is located, and number of rooms in the hotel;
(C) Proof of ownership of the land/building; and
(D) Name, home address, business address, home phone number, business phone number, mobile phone number and facsimile number of each manager. (Ord. 22-03, 5-2-2022)
