To obtain a permit to operate a taxicab in the city, an applicant shall make a sworn written application to the City Manager, or the person designated by the City Manager, which application shall contain the following information:
(A) The name, date of birth and address of applicant.
(B) The name of the certificate holder by whom the applicant expects to be or is employed.
(C) The applicant's place of residence for the past five years.
(D) A full description of the applicant, including height, weight, color of eyes and hair, distinguishing body features or marks, an impression of the applicant's fingerprints taken by the Police Department and three prints of the applicant's photograph.
(E) Whether the applicant has had experience driving a taxicab in a related type of work.
(F) Whether applicant has been finally convicted of any or misdemeanor involving moral turpitude.
(G) A medical report, dated not more than 30 days prior to the date of application, from a physician certifying that applicant has been examined and does not suffer from any disease or disability which would impair his/her ability to operate a taxicab. The medical report shall specifically state the condition of hearing and eyesight of the applicant.
(H) The number and date of expiration and issuance of applicant's Texas Class A, B or C license.
(I) The most recent date of completion of a certified defensive driving course, if any.
(J) A list of all traffic offenses with which the applicant has been charged since receiving his/her driver's license, including offense, approximate date and disposition.
