The foregoing operating rate and surcharges shall be billed and collected monthly. Billing for the rates and charges shall commence on the sixteenth day of the month in which connection is made on or after the first day but before the sixteenth day of the month, and as of the first day of the month following connection where connection is made on or after the sixteenth day of the month. All the rates and charges shall become due on the twenty-fifth day of the month following the end of each billing period, and if the rates are not paid on or before that date, then a penalty of 5% per month shall be added thereto.
