Whenever an operator has lost or destroyed his/her village issued certificate, he/she shall submit an affidavit in writing to the Village Administrator, stating that the village issued certificate has been lost or destroyed. The Village Administrator shall issue a duplicate copy or substitute thereof upon payment of the sum $5 for each item so duplicated or substituted.
